Hiver 1917. Le docteur Bomgard apprend que son ancien camarade et condisciple Poliakov, médecin dans un village perdu d'une lointaine province de Russie, est en détresse et l'appelle à l'aide. Publié en 1927, ce court récit semi-autobiographique, journal d'une inexorable descente aux enfers tandis qu'au-dehors gronde la Révolution, est un des plus saisissants chefs-d'œuvre de Boulgakov.
